THE BASICS

What Is Interventional Psychiatry?

Most psychiatric care relies on medication and talk therapy. Interventional psychiatry adds treatments that act on the brain more directly: brain stimulation with transcranial magnetic stimulation (TMS), and medicines that work on the brain's glutamate system, such as esketamine and ketamine, given under monitoring in the clinic.

These treatments are usually considered after depression has not responded to at least two antidepressants. The National Institute of Mental Health describes TMS and other brain stimulation therapies as options when other treatments have not worked.

What sets interventional psychiatry apart is the setting. Every treatment happens in our office, with a psychiatrist setting the plan, a clinical team monitoring you, and standard rating scales tracking whether it is working.

Regulated Treatments

TMS devices are FDA-cleared, and Spravato is an FDA-approved medicine. IV and IM ketamine for depression are off-label, and we explain what that means before you start.

TREATMENTS

Interventional Treatments We Offer

We offer the three main interventional treatments for depression in one office, so your psychiatrist can recommend what suits you rather than whatever is available.

BRAIN STIMULATION

TMS Therapy

Magnetic pulses stimulate the brain area involved in mood. FDA-cleared for depression that has not responded to antidepressants. Sessions take about 20-30 minutes, with no anesthesia. About TMS therapy.

NASAL SPRAY

Spravato® (Esketamine)

An FDA-approved nasal spray for treatment-resistant depression, taken in our REMS-certified office with at least two hours of monitoring. About Spravato in NYC.

IV / IM

Ketamine Therapy

Ketamine given by infusion or injection, used off-label for depression and usually self-pay. You are monitored throughout and need a ride home. About ketamine therapy.

WHO IS REFERRED

Who Is Referred for Interventional Psychiatry?

Most of our patients are adults with major depression that has not improved after two or more antidepressants taken at a proper dose for long enough. Some come to us on their own after reading about TMS or Spravato. Others are referred by their psychiatrist, therapist or primary care doctor.

We also see people who responded to medication but could not tolerate the side effects, people with depression and significant anxiety, and people who would rather not add more medicines. Everyone has a full evaluation first, because these treatments are not right for everyone.

Clinicians can refer patients through our provider referral page. With the patient's consent, we keep the referring clinician updated and coordinate ongoing care with them.

MEASUREMENT-BASED CARE

How We Measure Progress

Baseline

Before treatment, you complete standard questionnaires, such as the PHQ-9 for depression, so we have a clear starting point.

During Treatment

We repeat the questionnaires at set points and ask about side effects, sleep and day-to-day life.

End of a Course

We compare your scores with your baseline and decide with you whether to stop, continue or change approach.

Follow-Up

We check in after treatment ends and plan maintenance if symptoms start to return.

SAFETY

Safety and Monitoring

TMS is given while you are awake and alert. The most common side effects are scalp discomfort and headache in the first sessions. Seizures are rare, and we screen for risk factors before you start.

Spravato and ketamine can cause dissociation, dizziness, nausea and a temporary rise in blood pressure, so we check your blood pressure and monitor you after each dose. Spravato is only given under the FDA-required SPRAVATO REMS program, with at least two hours of monitoring.

Safety steps we take

Every treatment plan is set and supervised by a board-certified psychiatrist.

Common Questions About Interventional Psychiatry

Everything you need to know before your free consultation.

What is interventional psychiatry?

It is a branch of psychiatry that uses procedures and closely monitored treatments, such as TMS, Spravato (esketamine) and ketamine, for conditions like depression that have not responded to medication and therapy. The treatments happen in a clinic, under a psychiatrist’s supervision.

Is interventional psychiatry the same as ECT?

No. ECT is one interventional treatment, but we do not offer it at Village TMS. We provide TMS, which needs no anesthesia, along with Spravato and ketamine. Our TMS vs ECT guide explains the differences.

Do I have to stop my current medication?

Usually not. Many people continue their antidepressant during TMS, and Spravato is often used together with an oral antidepressant. Your psychiatrist reviews every medicine you take, since a few can interact with these treatments.

Do I need a referral?

No referral is needed to contact us, though some insurance plans require one for specialist care. Clinicians can refer patients through our provider referral page.

Is interventional psychiatry covered by insurance?

TMS and Spravato are often covered when you meet your plan’s criteria, usually after prior authorization, which we handle. IV and IM ketamine are off-label for depression and usually self-pay. We verify your coverage for free.

How do I know which treatment is right for me?

That is what the evaluation is for. Your psychiatrist weighs your history, the side effects you want to avoid, your schedule and your coverage, then explains the options. Our treatment-resistant depression page has a side-by-side comparison.

Where are treatments given?

At our office at 80 Fifth Avenue, Suite 1405, New York, NY 10011, at 14th Street near Union Square. Every interventional treatment takes place in person.
