SCIENCE-BACKED TREATMENT

What Is TMS Therapy?

TMS — Transcranial Magnetic Stimulation — is an FDA-cleared, non-invasive brain stimulation therapy used to treat major depressive disorder, OCD, anxious depression, and other conditions. Approved by the FDA for depression since 2008, TMS therapy has helped tens of thousands of patients across the United States, including many here in New York City, who did not respond adequately to antidepressant medications.

Each TMS session uses a small treatment coil placed gently against your scalp to deliver brief, precisely targeted magnetic pulses. These pulses stimulate underactive neurons in the prefrontal cortex — the region of the brain most consistently linked to depression and mood regulation. The process is the same physics principle used in MRI scanners, applied at lower frequency and in a highly targeted way.

TMS is fundamentally different from antidepressant medications. Medications work systemically, meaning they circulate throughout your entire body and can produce side effects such as weight gain, sexual dysfunction, fatigue, or nausea. TMS has no systemic effects whatsoever — no drug enters your bloodstream. It targets specific brain circuits directly, without touching the rest of your body.

TMS also differs meaningfully from electroconvulsive therapy (ECT). ECT requires general anaesthesia, induces a controlled seizure, and is associated with short-term memory loss. TMS requires no anaesthesia, produces no seizure, and has no known effects on memory. Patients remain fully awake during every TMS session, can drive themselves to and from the clinic, and return immediately to their normal day.

Science-Backed & FDA-Cleared

TMS has received FDA clearance for major depressive disorder, OCD, anxious depression, and smoking cessation. Decades of peer-reviewed clinical research support its safety and efficacy.

OUR APPROACH

TMS Treatment We Offer

We offer IV, IM, and Spravato under one roof, so your psychiatrist can match the
right approach to your situation.

FDA-CLEARED

Standard rTMS

Repetitive TMS (rTMS) uses a figure-8 coil to deliver focused magnetic pulses to the left dorsolateral prefrontal cortex. This is the primary FDA-cleared protocol for major depressive disorder and the most extensively studied form of TMS therapy in clinical literature. It is the foundation of treatment for most patients with depression.

FDA-CLEARED OCD & MORE

Deep TMS (dTMS)

Deep TMS uses an H-coil to reach deeper and broader brain circuits than standard rTMS. It holds FDA clearance for OCD, anxious depression, and smoking cessation, in addition to major depression. For patients whose primary challenge is OCD or treatment-resistant anxious depression, dTMS provides a more targeted therapeutic pathway.

IV INFUSION

Intramuscular (IM) Injections

Theta-burst stimulation (TBS) delivers the same therapeutic magnetic targeting in significantly shorter session times — sometimes under 10 minutes — while maintaining comparable efficacy to standard rTMS protocols. Accelerated TMS schedules allow multiple sessions per day, compressing the overall treatment course for patients who need a faster pathway to relief.

THE PATIENT JOURNEY

What to Expect During Treatment

Starting TMS is straightforward. Here is a clear, honest account of
what your treatment course at Village TMS looks like from the first
session through completion.

Your First Session: Brain Mapping

Before your first treatment pulse is delivered, your clinician performs motor threshold determination — a brief, precise calibration process that identifies the optimal magnetic intensity for your individual brain anatomy. A small test pulse is used to locate your motor cortex, and settings are adjusted from there. This personalisation step ensures your protocol is tailored to you, not applied as a generic setting. It adds approximately 15–20 minutes to your first session only.

During Each Session

You sit in a comfortable, reclining treatment chair. The TMS coil is positioned against the left side of your scalp. You will feel a light tapping or clicking sensation at the treatment site — most patients describe it as a noticeable but manageable rhythm. Sessions last approximately 20 to 30 minutes, depending on the protocol. You are free to read, listen to music, use your phone, or simply rest. No sedation is required. When the session ends, you stand up, collect your belongings, and drive yourself home. Most patients schedule sessions in the morning and go directly to work afterward.

Your Course of Care

A standard course of TMS at Village TMS consists of approximately 36 sessions delivered over roughly six weeks — around five sessions per week. This schedule follows the evidence-based treatment protocol and can be adjusted based on your clinical response and schedule. Some patients begin to notice shifts in mood, energy, or sleep as early as weeks two or three; others experience the most meaningful changes in weeks four through six. Your psychiatrist tracks progress with validated clinical measures throughout and adjusts your protocol as needed.

After Your Intiial Course

When your initial treatment course ends, your psychiatrist will evaluate your response and discuss next steps. Some patients benefit from periodic maintenance sessions — scheduled as clinically indicated — particularly if symptoms begin to re-emerge over time. There is no dependency, no withdrawal, and no systemic medication to manage. Many patients remain in remission for an extended period after a single course, and return for a booster course if and when they feel it is needed.

SAFETY & TRANSPARENCY

Is TMS Safe? Side Effects & What to Know

TMS therapy has an excellent safety record built over more than 20 years of clinical use and extensive peer-reviewed research. Here is an honest account of what the evidence shows — including the rare risks and who TMS is not appropriate for.

The most commonly reported side effect is mild scalp discomfort or a localised headache at the treatment site. This typically occurs during the first few sessions as your scalp adjusts to the sensation, and tends to decrease significantly — or disappear entirely — within the first week of treatment. Over-the-counter pain relief is effective if needed.

Unlike many antidepressant medications, TMS produces no systemic effects. There is no weight gain, no sexual dysfunction, no fatigue, no nausea, and no sleep disruption associated with TMS itself. And unlike ECT, TMS carries no risk of anesthesia complications and no associated memory loss — one of the most important distinctions for many of our patients.

The risk of seizure during TMS is very low — lower than that associated with several widely prescribed antidepressant medications. Because TMS uses focused magnetic energy near the scalp, it is not appropriate for individuals with certain metallic implants in or near the head — such as cochlear implants, deep brain stimulators, or aneurysm clips. This is why we conduct a thorough clinical creening before every course of treatment begins. Your safety is always the first priority.

No Systemic Side Effects

No weight gain, no sexual side effects, no fatigue — none of the systemic effects associated with antidepressant medication.

No Memory Effects

Unlike ECT, TMS does not affect memory. Patients consistently report no cognitive changes attributable to TMS.

Very Low Seizure Risks

The risk of seizure is very low. Thorough pre-treatment screening identifies and excludes any patients for whom TMS is not appropriate.

Screening Is Essential

TMS is not appropriate for patients with certain metallic implants near the head. Our pre-treatment screening process ensures you are a safe candidate before any treatment begins.

Common Questions About TMS Therapy

Everything you need to know before your free consultation.

What Does TMS Stand For?

TMS stands for Transcranial Magnetic Stimulation, an advanced therapy for mood disorders that uses a TMS machine to deliver safe magnetic pulses to the brain. This approach stimulates underactive areas of the brain linked to mood regulation, helping restore balance and improve emotional health. Many patients turn to TMS when traditional antidepressant medications have not provided relief.

What does "treatment-resistant" mean?

A condition is labeled “treatment-resistant” if symptoms persist despite trying multiple evidence-based therapies. For example, depression is considered treatment-resistant after failing at least two antidepressant medications (at adequate doses and durations) combined with psychotherapy. This doesn’t mean you’re “out of options”—it simply guides us toward advanced therapies like TMS.

Is TMS an Outpatient Procedure?

Yes, TMS is an outpatient procedure, performed at our Medical Center, allowing you to return to work or school immediately after your appointment. Each session lasts about 15–20 minutes, and no anesthesia is required. Because there is no recovery time, it fits easily into most daily routines.

How Does TMS Compare to Antidepressant Medications?

Unlike antidepressant medications, TMS directly stimulates nerve cells in the prefrontal cortex. This targeted approach reduces the side-effects of TMS while addressing the root cause of mental health conditions. For patients with treatment-resistant depression, TMS can be an important next step when medications alone are not effective.

Can I continue my current medications during TMS?

Yes! TMS can be safely combined with most medications. In fact, many patients use it alongside antidepressants or mood stabilizers. Never stop or adjust medications without consulting your prescribing provider first.

How Many Sessions Are Required?

A full treatment course usually includes 20–36 sessions. Each course of treatment is customized to your unique needs. While some patients notice changes within just a few weeks, others may need the full series of treatments to experience maximum benefit.

Where Can I Find TMS Clinics Near Me?

What Insurance Providers Do You Accept?

We work with most major carriers, including United Healthcare, and serve as a network provider. Our team will coordinate with your insurance provider to ensure smooth access to care. While coverage varies, many patients are pleasantly surprised to learn that benefits are available for TMS treatment. Many patients find that their plan may include insurance coverage for TMS therapy. Our team can provide guidance and help you check directly with your insurance provider to confirm whether benefits are available for your course of treatment. Because every plan is different, verifying coverage is the first step toward determining your treatment pathway.

Will I need maintenance sessions after my initial treatment?

Some patients opt for periodic “booster” sessions to sustain results, similar to follow-up therapy or dental cleanings. We’ll create a personalized maintenance plan based on your progress and long-term goals.

Does TMS Help With Conditions Beyond Depression?

Yes. In addition to Major Depressive Disorder, TMS Right has shown promise in reducing symptoms of chronic pain, obsessive-compulsive disorder, and Bipolar Disorder. Ongoing clinical trials continue to explore its benefits for a wide range of mental health conditions. This makes TMS one of the most exciting and versatile advances in modern Interventional Psychiatry.

What makes TMS different from ECT (electroconvulsive therapy)?

Unlike ECT, TMS requires no anesthesia, doesn’t induce seizures, and has no risk of memory loss. It’s gentler, with minimal downtime, making it easier to integrate into daily life.

How do I prepare for my first session?
  • Avoid caffeine beforehand (it can heighten scalp sensitivity).

  • Remove magnetic-sensitive items (e.g., jewelry, credit cards).

  • Wear comfortable clothing—you’ll relax in a chair during treatment.
